This intake includes acknowledgment of risk, release of liability, emergency medical authorization, health and fitness confirmation, participant responsibilities, media release, communicable illness awareness, indemnification, proper equipment requirement, and Pennsylvania governing law.

Submission requires waiver confirmation and consent agreement before participation can be approved.
